Congratulations! It is a long hard road, but you have taken the most difficult step on your own - YOU LEFT! And that is something you should be proud of. That took a lot of courage, not many women would have done the same. So, be proud of what you have accomplished.
Now for the other stuff. It is overwhelming, but not impossible.
First of all call NACA - their link is below. They should be able to help with the mortgage.
2nd - apply for SSD - You said you were diagnosed with bipolar and an array of health problems - you may be able to get SSD and your son as well. If you are approved, your children may be able to get dependent benefits. You may be able to get a probono atty to assist with filing for SSD as well.
For the college expenses, apply for FAFSA - link below - due to your income, you may be able to get the kid's college expenses covered.
For a vehicle- try ways to work - they may be able to help you get a loan for a vehicle to get you where you need to go.
Now, none of this is guaranteed, but it is all worth a try. Give it a shot and let me know how it goes.
